MassHealth and Commonwealth Care Tobacco Cessation Benefits
Effective July 1st, 2006, Medicaid recipients in Massachusetts have access to all FDA-approved smoking cessation medications and counseling support.
Effective July 1st, 2012, Commonweatlh Care members in Massachusetts have access to all FDA-approved smoking cessation medications and counseling support.
The Massachusetts Tobacco Cessation and Prevention Program, MassHealth and Commonwealth Care have developed the tools below to help providers and their patients to understand and use these benefits.

UMass Medical School Tobacco Use Intake and Assessment Form
The Center for Tobacco Treatment Rsearch and Training (CTTRT), with the Massachusetts General Hospital Tobacco Research and Treatment Center, developed an Intake, Assessment, and Treatment Planning guide to assist providers in conducting a thorough patient assessment and developing a treatment plan.
The Guide shows providers how to use this tool and includes a blank template, a case study with a sample completed intake and assessment form, and a quick guide to pharmacotherapy.
